Dr. Callum Cowan runs Phenom High Performance Medicine — a health optimisation practice for elite professionals. Each patient completes a detailed performance assessment, and based on their results they should receive a fully personalised training and recovery program. The problem: Dr. Cowan was reviewing every submission by hand, scoring it manually, then emailing out the correct program from a folder of documents. This process was consuming hours every week and didn't scale. He needed a system that could take a patient from assessment to receiving their personalised program without any manual intervention.
A fully automated assessment-to-delivery ecosystem
A complete end-to-end automation built across Typeform, Make (Integromat), Google Sheets, and Kajabi. The patient completes a 16-question Typeform assessment. Make captures the responses, runs them through a scoring formula in Google Sheets to determine one of 8 performance profiles, then automatically enrolls the patient in the matching Kajabi course and sends a personalised email with access instructions — all within seconds of submission. A re-assessment module was also built to track progress over time.

Primary Make scenario capturing Typeform responses, writing data to Sheets, reading the profile result, and routing the patient to the correct downstream actions
8 complete Kajabi courses built and structured — one per performance profile — with lesson content, program materials, and branded product pages
Make triggers Kajabi enrolment via webhook the moment a profile is determined — the patient receives course access before Dr. Cowan even sees the submission
8 distinct email templates dispatched via Gmail through Make — each referencing the patient's name, profile type, and direct Kajabi login link
Live Sheets dashboard logging every patient submission, profile result, enrolment status, and assessment date — Dr. Cowan's single source of truth
Separate Make scenario for follow-up assessments — compares new results against baseline, detects profile changes, and triggers updated course access if needed
Complete written documentation covering every scenario module, scoring logic, and Kajabi structure — allowing Dr. Cowan to manage and expand the system independently
